Our client is seeking a Remote Patient Care Coordinator (Cardiology Patient Navigator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, telehealth, utilization review, or home health will be competitive. What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan.
