Responsibilities

→Provide mobile primary care visits to seniors
→Manage chronic and acute conditions
→Perform comprehensive assessments & care planning
→Collaborate with caregivers, facilities, and interdisciplinary teams
→Deliver compassionate, patient-centered care
→Maintain accurate EMR documentation

Requirements

Active NP or PA license
1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred
Strong communication and clinical skills
Valid driver’s license & reliable transportation
