We’re hiring a Clinical Liaison (Hospice) – a compassionate and collaborative professional who serves as an advocate for patients and families during the pre-admission process. This role bridges clinical, sales, and marketing teams to ensure a smooth transition into hospice care and supports the growth of our service line through strategic outreach.

Responsibilities
~1 min read- →Serve as an advocate for patients from pre-admission through discharge from medical facilities
- →Collaborate with sales, marketing, and clinical teams to source hospice referrals from healthcare providers
- →Review Explanation of Benefits (EOBs) and consent forms with patients and families
- →Perform comprehensive nursing assessments to evaluate hospice eligibility
- →Support the Hospice service line in developing and executing sales and marketing strategies
- →Promote Silverado’s mission and values through community engagement and public relations
Requirements
~1 min read- LVN or RN license preferred, but not required
- Knowledge and experience working with hospice patients and families, professionally or as a volunteer, highly preferred
- Effective communication and interpersonal skills
- Must pass a criminal background check, physical exam, and drug screening
- Valid driver’s license and clean driving record required
- Must comply with all state and federal driving regulations
- Reliable transportation required
