Transition Advisor is a part-time (5-10 hours/week) hourly role earning $19/hour

Responsibilities

→Manage caseload with required one-to-one sessions
→Implement and modify curriculum for classes
→Facilitate and/or Co-Facilitate classes based on setting goals, navigating resources, and increasing independence
→Provide personalized support that is aligned with programmatic goals and outcomes
→Complete monthly evaluations for each individual on caseload
→Maintain and update activities and resources focused on transition-aged individuals

Requirements

Bachelors degree in social work, special education, non-profit management, or related field preferred
Two or more years of experience working with transition-age individuals with intellectual and/or developmental disabilities preferred

We look forward to welcoming someone with the following knowledge and skills:

Willingness and ability to learn various technology platforms and effectively teach participants how to use them
General knowledge of special education services and post-high school options
Strong understanding of effective ways to support teens and young adults with disabilities
Strong relationship-building skills with individuals, families, and colleagues
Experience leading and guiding teens and young adults in both individual and group settings
Confidence facilitating curriculum for both small and large groups
Resourcefulness in navigating technology, applications, and community-based resources
