The position

As a Support Coordinator you will be passionate about empowering individuals to achieve their goals and live fulfilling lives? Join our Disability Services team in Wangaratta.  With us, you’ll combine your strategic thinking abilities with a hands-on, outcomes focused approach. At VMCH, the future is bright and rewarding. In this role, you will work collaboratively with participants, families, carers, and service providers to help individuals navigate the NDIS and access the supports they need.

The responsibilities

  • You are experienced in the Disability sector with strong knowledge of the NDIS.
  • You will collaborate with participants, informal supports, and stakeholders to deliver person-centered support.
  • Provide coaching and guidance to help participants navigate the NDIS and community resources.
  • Organise and facilitate meetings with participants, families, and service providers. 
  • Monitor budgets and ensure services align with participants’ plans and goals.
  • Respond to service gaps or crises, coordinating immediate interventions as needed.

What’s required

  • Experience within the Disability sector or a relevant qualification in disability or community services
  • Strong knowledge of NDIS, Disability Act and other relevant legislation
  • Demonstrated experience in identifying, measuring and reporting on client and program outcome’s
  • Ability to work autonomously and cooperatively as a member of a team and manage and prioritise competing demands
  • Demonstrated ability to establish and maintain positive professional relationships with stakeholders.
  • Excellent communication skills both written and verbal
  • Current NDIS Screening Clearance and Working with Children’s
