Social Services Director

• →Complete individualized social histories, assessments, and care plans for residents upon admission.
• →Identify residents’ social, emotional, and psychological needs and establish appropriate goals and interventions.
• →Participate in interdisciplinary care plan meetings and collaborate with the healthcare team to provide comprehensive resident care.
• →Maintain timely and accurate documentation, progress notes, and records in accordance with facility policies and state and federal regulations.
• →Assist residents and families with discharge planning, placement, and transitions of care.
• →Provide guidance, advocacy, and emotional support to residents and their families throughout their stay.
• →Organize and facilitate family meetings and group sessions to promote communication, education, and support.
• →Educate residents, families, and the community about aging, resident rights, available services, and long-term care resources.
• →Assist with pre-admission screening and support prospective residents and families throughout the admission process.
• →Help new residents and their families adjust to the long-term care environment.
• →Maintain accurate social services records and manage department supplies and budget in accordance with company policies.
• →Collaborate with community agencies and outside resources to connect residents and families with appropriate services.

• Minimum of 2 years of social work experience; healthcare or long-term care experience preferred.
• Bachelor’s Degree required; Master’s Degree (preferred)
• Strong knowledge of social services practices and applicable state and federal healthcare regulations.
• Excellent written and verbal communication skills.
• Strong interpersonal skills and the ability to build trusting relationships with residents and families.
• Ability to work effectively as part of an interdisciplinary healthcare team.
• Comfortable using computers, electronic documentation systems, and other workplace technology.
• Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
• A compassionate, resident-centered approach and genuine desire to make a difference.
