As Service Integration Coordinator, you’ll help establish a new statewide mental health service by building relationships and referral pathways that connect people to the right support at the right time. You’ll map health and community services, connect with local organisations, support community engagement and maintain the Connected Care Support Catalogue.
This is a practical, relationship-focused role with variety and the opportunity to help shape how a new ‘no wrong door’ service works across Tasmania.
Connected Care is a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania.
The service supports adults aged 18 years and over to access free, coordinated mental health and wellbeing support, using a stepped-care approach that responds to their individual needs and goals. A multidisciplinary team brings together clinical, psychosocial and peer expertise to support people to access the right care and move between levels of support as their needs change.
About the Role
~1 min readThis role will help build the foundations for Connected Care across Tasmania. Working with the Service Lead and Connected Care team, you’ll:
- Map and maintain information about health, mental health and community services
- Develop and maintain referral pathways and relationships with key organisations
- Attend community and interagency networks and deliver information sessions
- Support engagement with diverse and underserved communities
- Contribute to resources, reporting and the evaluation of referral pathways
- You’ll work across a broad network including mental health services, general practice, primary and allied health, community organisations, housing and homelessness services, family and carer supports, and services supporting diverse communities.
The role can be based in Tasmania’s South with regular travel across the region and some travel elsewhere in Tasmania.
You’ll bring experience building strong working relationships across health, mental health or community services, with the ability to connect people and organisations and support the development of practical referral pathways.
You’ll have an understanding of community development and recovery-oriented, trauma-informed and strengths-based approaches, along with knowledge of the broader services that support people's mental health and wellbeing.
You’ll also bring:
- A relevant tertiary qualification, or a Certificate IV/Diploma with relevant experience
- Confidence engaging with a wide range of stakeholders, including presenting information to groups
- An understanding of culturally safe and inclusive practice and the diverse communities accessing health and community services
- An understanding of family and caring roles and their impact on carers' wellbeing
- Strong organisation skills and the ability to manage your own work and deliver against an agreed plan
- The ability to gather, interpret and use information and feedback to identify issues and opportunities
- Confidence preparing clear written information, resources and guides
- Good Microsoft Office skills and the ability to learn new systems
- A willingness and ability to travel throughout Tasmania
- Experience working within the Tasmanian health and community service system, or developing service directories, resource guides or referral pathways, will be highly regarded.
- Personal lived experience of mental health challenges, or caring for someone who has, is also welcomed.
