Home Health Physical Therapist (PT) Full-Time

• Pay: Negotiable based on experience, knowledge, skills, and abilities
• Location: Bushnell (Sumter Co./Citrus Co.)
• Schedule: Full-Time

• Mileage: Currently $0.60 per mile
• Reports to: Branch Director

• →Assess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environment
• →Carry out the physical therapy plan of care set by the physician
• →Instruct patients and families on therapeutic appliances and prosthetic devices
• →Document care accurately and on time, including clinical and progress summaries
• →Coordinate with physicians and report patient response to treatment or changes in condition
• →Identify needs for other home health services and refer as appropriate
• →Participate in discharge planning and case conferences
• →Provide consultation to families/caregivers and inservice education to organization personnel
• →Supervise physical therapy assistants per organization policy and state regulations
• →Check in on home health aide care every two weeks when therapy is the only skilled service
• →Take on other duties as assigned

1. Possesses a degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.
2. Licensed to practice as physical therapist within the state, if applicable.
3. Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
4. Demonstrates good verbal and written communication and organization skills.
5. Possesses and maintains current CPR certification.
6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Required before providing patient care:

• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
• Documented negative TB test (TST or TB blood test) taken within the past 12 months
• TB risk assessment and symptom evaluation upon hire

This role requires the following, with or without reasonable accommodation:

• Prolonged walking and standing
• Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
• Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
• Reaching, stooping, bending, kneeling, and crouching
• Visual acuity and hearing sufficient to perform required duties
