Conducts pre-visit chart reviews of patients, identifying needed preventive health maintenance and immunizations. Completes daily chart preparation prior to patient appointments. Performs medical record reviews, data collection, chart abstractions, reporting, and analysis. Coordinates and performs onsite medical record reviews. Identifies patients overdue for visits, labs, or referrals, and arranges follow-up services to assist with care gap closure. Assists in developing and implementing frontline practice strategies to address and close care gaps and increase compliance with preventive care recommendations. Researches best practices, national and regional benchmarks, and industry standards of care. Collaborates with leadership to produce reports, and evaluates utilization and quality performance indicators. Monitors for adverse trends and recommends modifications and corrective actions. Participates in relevant committees and work groups, making presentations and preparing reports, data, or other materials for presentations. Other duties as assigned.

  • Drive for Results: Translate strategic priorities into operational reality; create a culture of accountability and ensure performance standards are met; define and monitor processes to foster results; establish and develop sign-off points and best practices to maximize results. Plan, organize, and prioritize work with attention to detail to achieve goals and objectives [Required]
  • Focus on Consumers: Ensure that the consumer perspective and quality healthcare is a driving force behind activities; craft and implement service practices that meet consumers’ needs and organization values [Required]
  • Ability to work independently and with the team [Required]
  • Adaptability and flexibility [Required]
  • Strong computer skills in Microsoft office [Required]
  • Understanding of care gap closure and value-based care [Preferred]
  • CPT and ICD coding [Preferred]
  • Understanding of HEDIS, HCC, RAF requirements [Preferred]
  • Knowledge of health insurance and managed care industry [Preferred]
  • Previous experience with electronic medical records [Preferred]
  • Associate [Preferred]
  • High School Grad or Equiv [Required]
  • in a health related field
  • 1+ experience in a healthcare environment [Preferred]
  • American Association Medical Assistants (CMA) [Preferred] OR Clinical Medical Assistant Certification (CMAC) [Preferred] OR Registered Medical Assistant (RMA) [Preferred] OR National Certified Medical Assistant (NCMA) [Preferred] OR Certified Clinical Medical Assistant (CCMA) [Preferred] OR Nationally Registered Certified Medical Assistant (NRCMA) [Preferred]
