Responsibilities

- →Develop and implement individualized care plans that address emotional, social, and spiritual needs
- →Provide counseling and support to patients and families throughout the hospice journey
- →Document all interactions and maintain accurate patient records
- →Participate in interdisciplinary team meetings and collaborate with other care providers
- →Facilitate referrals to community resources and support services as needed
- →Provide bereavement support and counseling as required
