Responsibilities

  • Develop and implement individualized care plans that address emotional, social, and spiritual needs
  • Provide counseling and support to patients and families throughout the hospice journey
  • Document all interactions and maintain accurate patient records
  • Participate in interdisciplinary team meetings and collaborate with other care providers
  • Facilitate referrals to community resources and support services as needed
  • Provide bereavement support and counseling as required
