Licensed Practical Nurse, LPN, Full Time

As a Home Health LPN, you will:

• →Follow established care plans developed by the Registered Nurse (RN)
• →Perform skilled nursing visits in patients' homes
• →Assess patients and monitor their condition
• →Administer prescribed medications and treatments
• →Provide wound care and disease management education
• →Reinforce patient and caregiver teaching
• →Accurately document patient care and clinical observations
• →Communicate changes in patient condition to the supervising RN and physician
• →Collaborate with an interdisciplinary team to ensure optimal patient outcomes

• Current Licensed Practical Nurse (LPN) license in NJ
• Minimum one year of nursing experience required
• Previous home health experience preferred, but not required
• Strong clinical assessment and communication skills
