We are currently seeking a dedicated and adaptable Licensed Practical Nurse (LPN) to join our Beloit Regional Hospice team.

The primary responsibility of the Hospice LPN is to support the day-to-day activities of the hospice staff and clinical services. This includes clinical and clerical duties. The Hospice Licensed Practical Nurse provides information and services as a resource to others; achieves defined agency objectives in compliance with established and required guidelines. The primary focus of the position is to facilitate the highest quality of care through comprehensive and consistent support.

The Licensed Practical Nurse (LPN) will:

  • Exhibit each of the Beloit Health System Behavior Standards and role modeling excellence for all to see
  • Provide telephone calls and patient triage support
  • Supply and DME (durable medical equipment) ordering
  • Complete audits, documentation review, and related follow-up
  • Complete Patient visits as needed
  • Assist with and participate in marketing opportunities
  • Support RN case managers with patient needs and follow up to phone calls
  • Assist clinical manager with staff training, competencies, meeting minutes
  • Provide coverage for palliative care for vacation and ill-time coverage

Job Requirements

  • Graduate of accredited practical nursing program
  • Active Wisconsin LPN at time of hire
  • Illinois LPN preferred, if not active IL at hire, must apply for IL License within 30 days of hire
  • 2 years' experience in Palliative Care or Hospice preferred
  • Demonstrated ability to interact with wide variety of medical, professional and community groups
  • Computer proficiency and strong public speaking skills
  • Must have valid driver's license
  • Must have and maintain auto insurance coverage that meets requirements and maintains reliable vehicle
  • Ability to work independently and make autonomous decisions
  • Current CPR required
