Licensed Practical Nurse (Home Care) - PRN

• →Conducting client assessments and reassessments as assigned
• →Performing supervisory visits with clients
• →Ensuring compliance with agency policies, state regulations, and care plans
• →Communicating effectively with clients, families, and office staff
• →Providing documentation and reports in a timely manner

• Active Licensed Practical Nurse (LPN) license in good standing
• Experience in home care or community-based services preferred
• Strong communication and organizational skills
• Reliable transportation and ability to travel to client homes
• Ability to work independently and professionally
