Responsibilities
~2 min read- →Support the service user to connect with activities, support to sustain recovery and reach their goals set out in their aftercare plan e.g. training, employment support.
- →Facilitate post-treatment groups across the Bi Borough in community settings, outside of CGL hubs, to support individuals in progressing their recovery journey independently of services.
- →Provide service users with information on options available them within the service and in their communities.
- →Collaborate with the Partnership Lead to engage partner agencies that service users can access for support followingtreatment.
- →Build positive relationships with aftercare providers to ensure recovery supportis available to support post-Rehab early abstinence i.e., Build on Belief (BOB),, Fellowship Groups, Aspire2be, SMART
- →Work collaboratively with the Service User Inclusion Worker to understand posttreatment needs, ensuring the voices and preferences of service users areheard and reflected.
- →Working with Recovery Coordinators, Peer Mentors/Volunteers to provide Recovery Check-Ins
- →Excellent knowledge of inpatient detox and residential services
- →Experience of working within multidisciplinary teams and with fostering positive relationships with partner agencies and external providers
- →Experience of working with people at all stages of their treatment journey and an in-depth knowledge of the importance that groupwork and aftercare support offers
- →Excellent written and verbal communication skills and the ability to provide effective advocacy when needed

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
