Responsibilities

  • Conduct comprehensive patient assessments, including history taking and physical examinations across assigned clinic sites. 

  • Diagnosis and manage acute, chronic, and preventative health conditions in alignment with evidence based guidelines. 

  • Develop and implement patient care plans, including ordering and interpreting diagnostic tests, prescribing medications and initiating referrals as deemed appropriate. 

  • Maintain accurate, timely, and thorough documentation in the electronic medical record (Cerner). 

  • Collaborates with providers, care coordinators, medical assistants, and other interdisciplinary team members to ensure seamless care delivery. 

  • Participate in shared call with primary care providers.

Requirements

  • Completion of Masters in Nursing or Doctor of Nursing Practice from an accredited program 

  • Minimum of 2-3 years of Nurse Practitioner experience within a primary care setting 

  • Strong clinic assessment, critical thinking, and decision making skills

  • Excellent communication and interpersonal skills with patients and care team members
