Home Health Nurse- LPN/ RN

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

• Client Info: This is a sweet 3 month old who has a history of a congenital diaphragmatic hernia, VA ECMO, pulmonary edema, and feeding intolerance.
• Clinical Needs: Continued high-acuity care, including oxygen and NJ tube feeding.
• Environment: Clean pet-free home 

• →Provide skilled nursing care (trach/vent care, medication administration, catheter care)
• →Monitor and document patient condition and vital signs
• →Assist with daily living activities (ADLs)
• →Communicate with families and care teams
• →Maintain safe, high-quality, patient-centered care
  
  

• Full-time and part-time shifts available
• Day, night, and weekend shifts
• Weekly pay + optional daily pay

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills
