A Linkage to Care Specialist at Total Health Care facilitates access to medical and supportive services for vulnerable populations, particularly individuals newly diagnosed with or at high risk for HIV/AIDS and Hepatitis C. They focus on patient navigation, testing, and overcoming barriers to care
- Perform comprehensive assessments to identify psychosocial and medical needs. Develop individualized intervention plans using proven behavioral models like Motivational Interviewing and Harm Reduction.
- Attend community events, health fairs, and conduct targeted outreach to engage hard-to-reach or homeless populations.
Qualifications
- Education: A High School diploma or GED is required; an Associate’s or Bachelor’s degree in Public Health, Social Work, or a related field is highly preferred.
- Certifications: Maryland certification in Rapid HIV Counseling & Testing is typically required. Phlebotomy certification and Rapid Hep C training are often preferred.
- Experience: Experience working within the HIV/AIDS community, harm reduction, or social service agencies in Baltimore.
