Registered Nurse (RN) – Per Diem Medicare Home Health | OASIS Experience

Dedicated Home Health is growing in Palm Beach County and is seeking experienced Registered Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

What We're Looking For

Medicare Home Health and OASIS experience are strongly preferred.

Qualified candidates should have:

Current, unrestricted Florida Registered Nurse (RN) license
Medicare-certified Home Health experience
Hands-on OASIS assessment experience
Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits
Strong head-to-toe assessment and clinical documentation skills
Understanding of Medicare home health requirements and plan-of-care coordination
Ability to communicate effectively with physicians and the interdisciplinary care team
Reliable transportation and ability to travel within the assigned territory
Commitment to timely documentation and high-quality patient care

The Role

As a Per Diem Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.
