Job Title: Registered Nurse (RN) Case Manager – Home Health | Build Your Day, 1:1 Patient Care Company: Home Health Care, Inc. What You’ll Do • Manage a home health caseload across homes and community settings. • Complete OASIS assessments and individualized care plans. • Provide skilled nursing care, education, and coordination. • Collaborate with physicians, therapists, and interdisciplinary teams. • Support patients in maintaining independence and safety at home. • Maintain accurate, timely documentation. What We’re Looking For • Active Minnesota Registered Nurse (RN) license. • 3+ years of RN experience, home health preferred. • Strong clinical judgment and independence. • Organized, self-directed, and comfortable in a field role. • Reliable transportation.
