As the Admissions Nurse, you are the clinical quarterback, guiding the start of care and ensuring every patient begins with a clear, safe, and effective plan. This is ideal for nurses who want to step into a more strategic, assessment-driven role without moving into management. Why This Role Stands Out: • Own the admission process from start to finish, giving you full visibility and control over how each patient begins their care journey. • Step into a clinical leadership role in the field, making real-time decisions that shape outcomes without being tied to a desk. • Focus on high-level assessments rather than task-based visits, allowing you to fully use your clinical judgment and experience. • Be the trusted first point of contact for patients and families, setting expectations and building confidence in the care plan. • Work within an established, high-volume referral network, ensuring consistent opportunity and steady workflow. What You’ll Own: • Complete Start of Care (SOC) admissions, delivering thorough in-home assessments and accurate OASIS documentation. • Develop and initiate individualized care plans, ensuring each patient is set up for safe and effective home health services. • Identify risks, gaps, and service needs early, coordinating the right disciplines and resources from the start. • Communicate clearly with providers, caregivers, and internal teams to ensure alignment and continuity of care. • Set the tone for quality, compliance, and patient experience from the very first visit. What Sets You Apart: • Active Minnesota RN license with strong clinical foundation. • Proven experience with home health admissions and OASIS documentation. • Ability to think critically, prioritize quickly, and make confident decisions independently. • Strong communication skills with a focus on patient education and care coordination. • Reliable transportation and comfort working in a field-based environment. Additional Details: • Mileage reimbursement provided. • Territory-based scheduling across the Twin Cities Metro.
