The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

Schedule:

Per diem; 2 days a week or 6 shifts per month; 8:30am - 5:00pm

Location:

Contra Costa County & some parts of Alameda County

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Responsibilities

→One year experience as a professional nurse in the past three years or Bachelor’s degree in nursing from an accredited School of Nursing.
→Experience in acute care/ home health and/or hospice setting preferred.

Current license as a registered professional nurse in the State of California.
Public Health Nursing certification highly desirable.
HPNA certification preferred.

Pay Range:
$58.34 - $72.95 hourly
