The Resource Coordinator (RC) is responsible for providing support services for participants, developing Individual Anti-Violence plans (IAP), providing case management and guidance to participants through the reentry process while attending the JEVS SAS Violence Intervention Prevention program (VIP).

The RC will help prepare participants who have been referred by Philadelphia Department of Probations and Parole as well as those selected for the program from the Philadelphia Department of Prisons (PDP) and the Montgomery County Department of Prisons (MCDP) facility, who have been identified as being high risk of recidivation of violent crime or participate in violent behavior that are between 30-60 days of being released from custody. The RC will facilitate VIP focus groups as part of the JEVS SAS VIP Program. The RC will also provide referral and assistance with securing wrap-round support services as may be needed by the clients in the JEVS SAS VIP Program, or other related reentry, Violence Intervention Prevention support programs.


Responsibilities

  • Assists SAS clients with identifying potential barriers to employment.
  • Develop clients’ Individual Anti-Violence plans (IAP)
  • The RC will facilitate daily VIP focus groups as part of the JEVS SAS VIP Program.
  • Assists clients in reaching their goals in the program by providing retention services, stressing good attendance, punctuality, and promotion of a positive work ethic.
  • Provides regular, ongoing individual counseling to address barriers, encourages achievement of goals and ensures retention of clients.
  • Maintains DAP notes regarding clients’ progress and prepares statistical reports as required by the program.
  • Follows procedures and policies as specified by program regulations.
  • Meets and establishes rapport with support agency partners and referred customers within established timeframes.
  • Assesses customer needs and provides appropriate support, referrals, and participant follow up.
  • Facilitates coordination of services throughout the continuum of care.
  • Network and establish working relationships with appropriate personnel at the Philadelphia, Montco County Assistance Offices, Community Agencies, Mental Health and Drug and Alcohol Organizations and other agencies as needed.
  • Act as a referral agent, referring clients to external agencies and organizations for assistance with behavioral health, issues, housing, employment, legal or family related issues. And provide follow-up referral assistance.
  • Guide participants in learning how to independently resolve and avoid issues, problems involving violence.
  • Be on-call after works hours to intervene at the time of a violent occurrence if needed.

  •   High degree of interpersonal skills in motivating individuals to achieve success from recidivating or committing violent crimes or getting re-involved in violent behavior.
  • Provide excellent, prompt, and staff assisted customer service to customers referred to and served by their program
  •  Provide timely and accurate communication to partners (Program Director, Job Developers & Instructors)
  •   Serve as the RC for assigned customers and meet the regulatory requirements for eligibility, participation, case notes and follow up services, regarding employment, retention and termination.
  • Assist clients in becoming job search ready and job ready through the amelioration of personal challenges, including needed supportive services such as childcare, transportation, work clothing and counseling.
  • Maintain the integrity of all customer information, including electronic, hard copy and filing systems
  •   Work with the Integrated Job Development Team and its job developers to transition clients to appropriate employment within an identifiable time period.
  • Tracking and management of all documentation of client success stories, files and placement records.
