Cornwall Council - Adult Social Work - Community Team

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
Responsibilities

- Proven track record of excellent strengths-based practice in a social care environment.
- Ability to undertake, analyze, and translate complex assessments of need, risks, and strengths.
- Proficiency in recognizing and responding to behavior indicating resistance to change, ambivalence, or selective cooperation with services.
- Capability to deliver difficult messages and take immediate action when necessary.
- Ability to form strong professional relationships within a multi-disciplinary setting.
- High level of written and verbal communication skills.
You will work with team members and multi-disciplinary teams from other agencies to meet the needs of vulnerable adults, operating effectively within the operational team. You must be able to travel to the Watergate PCN area as required. You will ensure that the views and wishes of individuals and carers are central to decision-making, complete MCA and Best Interest decision-making, and, if necessary, complete COP applications and attend hearings. You will protect adults in need of support services, complete safeguarding enquiries, and intervene in cases of self-neglect. You will promote independence, prevention, re-ablement, and self-determination, facilitating effective communication with families, carers, individuals, and other agencies. It is essential to be based within travel distance of Watergate PCN or be able to travel daily into the area.
