Senior Adult Social Worker - Hammersmith and Fulham

• →Provide professional social work, occupational therapy, and other support to individuals, children, their families, and carers in Hammersmith and Fulham, in line with current national legislation and local policies and procedures.
• →Use professional judgment to employ a range of interventions promoting choice, control, and independence.
• →Demonstrate expert and effective practice in complex situations, assessing and managing higher levels of risk, balancing support and control, and liaising with a wide range of professionals, including more senior levels.
• →Operate autonomously, demonstrating expert and effective practice in complex situations, assessing and managing higher levels of risk, balancing support and control, and liaising with a wide range of professionals, including more senior levels.
• →Possess highly developed specialist knowledge of social work practice in adults with a mental health condition and be registered with Social Work England.
• →Assist individuals, carers, and families in developing support plans/care and treatment plans to meet identified outcomes/treatment goals, using a strength-based, asset-based approach and family, friends, and community options where appropriate and eligible needs are identified.
