Responsibilities

→Provide support and develop resilience and independence.
→Ensure that adults and their families are at the center of decision-making about their lives and the outcomes they want to achieve without compromising their safety.
→Develop an understanding of the culture, health, and social needs of the locality or community to inform practice.
→Undertake basic preventative work to reduce the need for managed care or accommodation.
→Make and support placement arrangements and future plans for individuals when necessary.
