Social Worker – Hospital Discharge

Liquid Personnel is seeking a dedicated Social Worker to join its client's Hospital Discharge Team in the London Borough of Richmond upon Thames. In this role, you will play a crucial part in ensuring safe, timely, and person-centred hospital discharges, helping adults return to their homes or community settings with the right care and support. This position promotes independence, wellbeing, and embraces a “Home First” approach.

In this role, you will:

• →Coordinate and manage the discharge process for adults with diverse and complex needs.
• →Work within the Discharge to Assess (D2A) framework to facilitate timely transitions.
• →Undertake Care Act 2014-compliant assessments, including Mental Capacity Act assessments and Best Interest decisions.
• →Develop strengths-based, person-centred care plans that promote independence and choice.
• →Collaborate with health professionals, therapy teams, housing providers, and voluntary sector partners.
• →Identify and respond to safeguarding concerns in line with statutory guidance and local policy.
• →Maintain accurate records and ensure adherence to relevant legislation (Care Act, MCA, DoLS).

To be successful in this role you must have:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
