Social Worker - Learning Disabilities and Autism Team

Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities and Autism Team at Coventry.
This is an excellent opportunity for a dedicated practitioner to support adults with learning disabilities and/or autism, delivering strengths-based, person-centred social work interventions that promote independence, wellbeing, and positive outcomes.
The successful candidate will manage a diverse caseload of adults with learning disabilities and autism, undertaking statutory assessments, safeguarding enquiries, mental capacity assessments, Court of Protection work, and support planning. You will work collaboratively with individuals, families, carers, health professionals, and partner agencies to ensure people receive appropriate support and services that maximise choice, control, and community inclusion.
Responsibilities
~2 min read- →Managing a caseload of adults with learning disabilities and/or autism, undertaking assessments and interventions in accordance with statutory duties and best practice.
- →Completing strengths-based Care Act assessments and reviews to identify eligible needs, outcomes, and appropriate support solutions.
- →Undertaking Mental Capacity Assessments and applying the principles of the Mental Capacity Act 2005 to support informed decision-making and protect individuals' rights.
- →Completing safeguarding enquiries and investigations under Section 42 of the Care Act 2014, ensuring individuals are protected from abuse, neglect, self-neglect, and exploitation.
- →Preparing reports, assessments, statements, and supporting documentation for Court of Protection proceedings when required.
- →Working proactively with individuals, families, carers, advocates, and partner agencies to develop person-centred support plans that promote independence and wellbeing.
- →Assessing and managing risk, ensuring appropriate interventions are implemented to safeguard vulnerable adults and promote positive outcomes.
- →Responding to urgent referrals and undertaking duty responsibilities as part of a rota system, including crisis intervention and risk management.
- →Supporting individuals through complex situations by utilising professional judgement, available resources, and relevant legislation.
- →Coordinating and reviewing care and support arrangements, ensuring services remain effective and responsive to changing needs.
- →Working closely with health professionals, housing providers, community organisations, and other stakeholders to ensure coordinated and integrated support.
- →Undertaking carers' assessments where appropriate and supporting carers to access relevant services and resources.
- →Maintaining accurate, timely, and comprehensive case records, assessments, and reports in line with legislative, professional, and organisational standards.
- →Promoting choice, independence, social inclusion, and community participation for adults with learning disabilities and autism.
Requirements

- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
