In this role, you will manage a caseload, conduct holistic strength-based assessments, and create personalized support plans. As a Social Worker, you will work within a multi-disciplinary team to support vulnerable individuals, aiming to prevent hospital admissions and help people remain in their homes. You must have knowledge of the Care Act, Mental Health Act, Human Rights Act, Mental Capacity Act, and associated Codes of Practice. Experience with the service user group and the ability to use social work methods to promote positive change, independence, and wellbeing is essential. Additionally, you need to apply relevant legislation to practice and possess good IT skills for accurate and timely case recordings.

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have at least 2 years of post-qualified experience.
