Job Summary: TEMPORARY 8/2/26 - 2/7/27. The Discharge Expeditor-RN is a highly skilled licensed registered nurse. That demonstrates advanced expertise and knowledge in the discharge process for complex patients, using independent clinical judgment to prioritize and coordinate timely transitions. Plays a significant part in optimizing patient flow across the hospital by proactively identifying, coordinating, and removing barriers to ensure timely discharges and patient progression of care. Partners closely with physicians, case management, social workers, bedside nurses, ancillary services, and leadership to ensure safe, efficient transition while maintaining high standards of quality, patient experience, and regulatory compliance. Maintains and actively monitors patient throughput dashboards to assess progress, identify trends, to drive data informed decisions hospital wide. Works closely with hospitality lounge staff, patient placement, nursing and physician teams to ensure timely and coordinated discharges of patients. Contributes to performance improvement initiatives that enhance patient flow, optimize capacity management, and reduce avoidable days. Serves as a clinical resource and coach to promote throughput best practices and safe discharge processes. Reviews, medical records and extracts/analyzes key performance indicators related to discharge delays. Produces clear, accurate, well designed reports and delivers timely, standardized reports to inform decision making, enhance flow, streamline the discharge process, improve patient satisfaction, and support financial performance. Establishes and leads a comprehensive discharge review process to analyze delays, identify root causes, and implement effective corrective actions. Facilitates ongoing multidisciplinary discussions to prevent recurrence and improve discharge efficiency. Serves as a project facilitator, using independent judgment and advanced critical thinking skills to support quality, safety, and strategic discharge initiatives. Guides data driven discussions focused on a high quality, efficient, patient centered care model that reduces cost and improves outcomes. Functions within the registered nurse scope of practice and established policies. Serves as an exemplary role model in patient care delivery methods and consistently demonstrates an advanced level of clinical practice, communication, and documentation. Facilitates the empowerment of the nursing profession and excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.
Education and Experience: Bachelor of Science Degree in Nursing required. Minimum three years of clinical experience in an a level one trauma center required. Masters Preferred. Minimum of one year of prior management or supervisory experience preferred.
Knowledge and Skills: Demonstrates comprehensive knowledge discharge process and barriers that slow down the process. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base on the latest trends in discharge processes, data bases for tracking. Basic knowledge of healthcare pay for performance, regulations and accreditation required. Good understanding of data base management, able to enter data and extrapolate date. Exhibits professional behavior at all times and works calmly and responds courteously when under pressure. Able to read, write legibly and speak in English (and Spanish preferred) with professional quality; use computer, printer and software programs necessary to the position, e.g., Microsoft Office, Outlook, electronic event reporting program, established organization electronic medical record; operate and troubleshoot equipment utilized for position; perform basic patient care activities using proper body mechanics and safety precautions; comprehend and follow-through with instructions; relate and communicate positively, effectively and professionally with others; be assertive and consistent in following and enforcing policies; collaborate; accept direction; think critically and use good judgment; problem solve and collaborate with licensed care providers as necessary; perform duties with minimal supervision; perform basic math functions; organize and prioritize workload; problem solve; recall information with accuracy; pay close attention to detail; distinguish colors as necessary for position; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; discern temperature variances through touch; see adequately to read computer screens, medical records and written documents necessary to position, exhibit professional behavior at all times.
Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS), certification issued by the American Heart Association required.
