Advanced Practice Provider (Float) - Geriatrics

We are hiring a Float Advanced Practice Provider (NP/PA) to support a large regional footprint — providing coverage for PTO, turnover, and new building launches while maintaining continuity of high-quality, relationship-based care.

This is a high-visibility role ideal for experienced clinicians who are adaptable, operationally strong, and interested in future leadership opportunities.

• Regional float coverage across home-based care and senior living communities
• Leadership-track potential (future pod ownership or regional growth roles)
• Autonomy with strong clinical and operational support
• Continuum-based care model: primary care, geriatrics, psych, and palliative integration

• →Provide mobile primary care to seniors in homes, boarding cares, ALF, SNF, and hospice settings
• →Step into established panels to ensure seamless patient continuity
• →Support new market expansion and building launches
• →Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)
• →Deliver dementia-informed, relationship-based care
• →Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)
• →Maintain strong documentation, communication, and quality standards

• Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license
• 2+ years experience in geriatrics, primary care, home health, SNF, or senior living preferred
• Comfortable practicing independently in mobile/community settings
• Strong clinical judgment and panel management skills
• Adaptable, organized, and growth-oriented
• Valid driver’s license and ability to travel within region
