We’re hiring a Director of Patient Care Services – a compassionate and experienced clinical leader who oversees hospice care delivery, supports interdisciplinary teams, and ensures regulatory compliance. This role blends leadership, clinical expertise, and empathy to provide dignified, meaningful care to patients and families.
Responsibilities
~1 min read- →Serve as a role model and resource for nursing staff and interdisciplinary team members
- →Support staff in problem-solving and determining appropriate clinical interventions
- →Collaborate with the Administrator on hiring, orientation, retention, and performance management
- →Adhere to budget requirements for labor, pharmacy, DME, and medical supplies
- →Coordinate patient care with RN Case Managers and attending physicians
- →Review and manage patients receiving higher levels of care (respite, inpatient, continuous care)
- →Oversee clinical and management responsibilities for assigned teams
- →Serve as liaison between Administrator, clinical staff, patients, families, and community partners
Requirements
~1 min read- Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field.
- Current, unencumbered Registered Nurse (RN) license in the state of employment.
- Active CPR certification required; certification in Hospice and Palliative Nursing preferred.
- Minimum of 4 years of experience within the last 5 years in hospice, palliative care, supportive care, home health, or a primary care setting.
- Minimum of 1 year of supervisory experience in a healthcare setting.
- Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable.
- Valid driver’s license with a clean driving record.
- Reliable transportation and willingness to travel as needed.
- Ability to lead bereavement support groups and make sound clinical decisions.
- Flexibility to work outside regular hours based on patient needs.
