We are hiring a RNLD Clinical Lead to join Thornbury Community Services who are an 'outstanding' CQC-rated care provider that deliver complex care at home or in the community.

In this role you will manage a caseload of packages to support individuals with learning disabilities, autism and mental health needs.

  • Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team.
  • Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained.
  • Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
  • Support the care team by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.
  • A learning disabilities nurse with an active NMC pin
  • Experience leading community teams to support individuals with learning disabilities, autism and mental health needs
  • A valid drivers license and access to a vehicle
