Position Summary
Job Status: Casual (2 positions)
Job Title: Home Support Worker
Hours of Work: On-Call as needed

Salary: $28.48 per hour
Reports to: Associate Health Director Programs

Internal / External Posting
The successful applicant will be required to work in accordance with the BCGEU Collective Agreement as well as the policies and benefits therein, in addition to all relevant Tla’amin Health policies and procedures. This position is open to applicants of all genders.
Key Responsibilities
Performs all duties and responsibilities in accordance with Tla’amin policies, standards, practices and procedures as directed by the Home and Community Care Coordinator. Maintains confidentiality as required pertaining to members, clients, employees and general matters of the Nation.
- Implements home support and personal care for clients as directed
- Works to ensure the social, spiritual health and well-being are met for all clients
- Assists clients with activities of daily living
- Maintains strict confidentiality as required pertaining to clients and their families
- Acts as a liaison for individuals in the Home & Community Care program
- Maintains accurate records of daily tasks, routine, and clients served
- Complete daily EMR charting of client information
- May be required to work outside normal office hours
- Transport and assists client(s) according to care plan
- Performs other duties as required
Qualifications
Training, Education, Experience
- Graduation from a recognized Health Care Assistant Program
- Registration with the BC Care Aide & Community Health Worker Registry
- Acceptable Criminal Record check, vulnerable sector screening
- Class 5 Driver’s License +clean drivers abstract
- CPR-C certification
- Current Food Safe certification
Knowledge, Skills, Abilities
- Knowledge of Indigenous health and wellness practices in Tla’amin
- Good organization and time management skills
- Knowledge of client documentation
- Demonstrated ability to work both independently and as a team
- Ability to communicate effectively both verbally and in writing
- Ability to work with an interdisciplinary team
- Physical ability to carry out the duties of the position
- Ability to prioritize and organize work
Working Conditions
Work is performed primarily in individual client homes. Occasional travel to attend meetings or training is required. This position may be required to work outside normal office hours depending on the needs of the individual clients.
Additional Information
All employees working for Tla’amin Health must be fully vaccinated for COVID-19. Proof of vaccination status will be required.
