About the role
We’re currently recruiting for a Care Partner to join our team in a full-time ongoing role.
As a Care Partner, you’ll play a vital role in supporting older adults to live independently, safely and stay connected within their homes and communities. Each day brings meaningful interactions and the opportunity to make a genuine difference.
Responsibilities

- →Conduct comprehensive assessments, develop care plans and coordinate services
- →Empower clients and carers to make informed choices about their care
- →Ensure compliance with aged care quality standards and organisational policies
- →Maintain accurate case notes, documentation and client confidentiality
- →Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required
About you
You are compassionate, adaptable, and genuinely committed to making a positive difference in people’s lives. With strong communication skills and a person-centered approach, you thrive in a role where every day brings new opportunities to support, empower, and connect with others.
- Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
- Experience as a Care Partner or Case Manager within Support at Home
- Excellent communication and computer skills
- Understanding of aged care reforms and quality standards
- Ability to work autonomously and manage competing demands
- Commitment to customer service, cultural diversity, and workplace safety
- Willingness to travel as required and a current Victorian driver’s licence
- Satisfactory National Police Check
- Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP)
- Experience working with people from diverse backgrounds
