What You’ll Own: • Complete Start of Care (SOC) admissions and OASIS documentation. • Perform comprehensive in-home assessments. • Develop and initiate personalized care plans. • Coordinate services and communicate with providers and caregivers. • Ensure safe, smooth transitions into home health services.
