Job Title: Registered Nurse (RN) Case Manager – Home Health, 1:1 Care & Autonomy Company: Home Health Care, Inc. What You’ll Do • Complete comprehensive nursing assessments and care plans. • Perform OASIS visits and coordinate care across settings. • Provide skilled nursing care, education, and support to patients and families. • Collaborate with physicians, therapists, and interdisciplinary teams. • Manage a caseload of patients in homes, assisted living, and community settings. • Maintain accurate, timely documentation and regulatory compliance. What We’re Looking For • Active Minnesota Registered Nurse (RN) license. • 3+ years of RN experience, home health preferred. • Strong assessment, care planning, and documentation skills. • Ability to work independently in a field-based role. • Reliable transportation and valid driver’s license.
