The Lead Mentor Associate will work in conjunction with the JEVS Independence Network team as the primary team member to provide additional support and assistance to the members of the program communities where needed.
- Communicates effectively with program members, parents, JEVS management, and all other team members.
- Takes the lead in tracking DLA-20 implementation as well as annual goal planning for the program utilizing data collected from the DLA-20 assessment tool. Solicits and incorporates input from Mentor Associates, Community Support Counselors, Career Navigators, members and families to support the development of meaningful annual goals. Works with the team and members to revise and adjust goals as needed throughout the year. Communicates changes to the team as needed.
- Reports directly to Associate Director. Reports any problems or concerns of the members and/or their families to the Director.
- Provides support to members, when appropriate, to participate in mentoring activities, including job-shadowing, interactive skills development; hands-on social role-playing and coaching.
- Facilitates group skills training sessions with members and documents observed progress and/or challenges.
- Leads the development of the Independence Network shared enrichment and activities calendar by researching and coordinating events; maintains Google calendar and ensures distribution to all members; assures monthly program event goals are met.
- Coordinates and tracks data captured for special projects, customer service surveys, and grant deliverables to report outcomes back to stakeholders, organizational leadership, and funders.
- Assists members in the community, provides mentoring support, assists with community participation, employment engagement, volunteering and travel training instruction when needed.
- Communicates with members via email, phone and in person. Offer members social support, and guidance in the career and exploration process, and assistance in setting goals aimed at achieving independence.
- Coordinates with the team to set goals with members; assist with tracking progress; revise as necessary to ensure that members receive meaningful and quality support aligned with personal priorities; communicate progress with families; provide guidance and recommendations as needed to all team members related to support strategies.
- Assists with outreach to promote the program through presentations and distribution of materials.
- Identifies potential community connections and partnerships that would benefit members and the program; coordinates opportunities to build relationships with community partners; attends meetings and serves on committees with or without members to actively build relationships in the community.
- Ensures changes in a person’s health and/or health management are communicated to staff on the team.
- Completes all paperwork in a timely and satisfactory manner.
- Adheres to all JEVS Policies and Procedures (Confidentiality, HIPAA laws, etc.)
- Maintains a minimum of 24 hours of professional and practical training annually (CPR-First, Crisis-Management, Principals of Self-Determination, etc.). Attends program specific trainings.
- In addition to the above noted tasks and responsibilities, other such related duties may be assigned from time to time, including on call (warmline) responsibilities. With consideration to the continuing development of the JEVS Independence Network programs, the employee recognizes the possibility of adjusting the responsibilities according to the needs of each program.
