Liquid Personnel is recruiting an Adult Social Worker for its client’s Hospital Social Work Team based in Birmingham.

As an Adult Social Worker in the Hospital Social Work Team, you will support adults transitioning from acute hospital care, helping them return safely to the community or move into appropriate care settings. You will play a key role in discharge planning, working closely with health professionals, patients, and families to ensure timely, person-centred outcomes.

You will be expected to apply a strengths-based approach, incorporating the Three Conversations Model to promote independence and well-being. The ideal candidate will demonstrate a proactive, solution-focused attitude, with experience in managing complex discharge pathways and working within multidisciplinary teams. This role requires strong skills in assessment, care planning, risk management, and safeguarding, along with the ability to perform under pressure in a fast-paced environment. A thorough understanding of the Care Act 2014, Mental Capacity Act 2005, and hospital discharge legislation is essential.

Responsibilities

  • Undertake timely assessments of adults being discharged from hospital, ensuring their needs are identified and met in line with the Care Act 2014.
  • Facilitate safe and effective hospital discharges, working collaboratively with health professionals, patients, families, and community services.
  • Apply a strengths-based approach using the Three Conversations Model to promote independence and reduce reliance on formal services.
  • Develop and implement person-centred care and support plans, ensuring they are outcome-focused and proportionate to the individual's needs.
  • Assess mental capacity and make best interest decisions in accordance with the Mental Capacity Act 2005.
  • Manage complex cases, including safeguarding concerns, delayed discharges, and individuals with multiple health and social care needs.
  • Work within a multidisciplinary team, contributing to integrated discharge planning and attending ward rounds, case conferences, and strategy meetings.
  • Maintain accurate and timely records, ensuring all documentation complies with legal and organisational standards.
  • Provide advocacy and support, ensuring individuals understand their rights and options throughout the discharge process.
  • Keep up to date with legislation, policy, and best practice, applying this knowledge to improve service delivery and outcomes.

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Substantial experience working with vulnerable adults, including those with complex needs.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
  • Skills: Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting.
  • Driving Licence: Full UK driving licence and access to a car is often required for community visits.
