Liquid Personnel is currently recruiting for an experienced Social Worker to join its client’s Integrated Cluster Network Team based in Bridgend, Wales.

This is an exciting opportunity to work within a multi‑disciplinary service that supports adults with long‑term, complex needs relating to physical health, mental health, and emotional well‑being. The team works with both younger and older adults, adopting a strengths‑based, person‑centred approach to ensure individuals are supported to live their lives with choice, independence, and control.

Responsibilities

  • Undertake strengths‑based, person‑centred assessments under the Social Services and Well‑being (Wales) Act 2014, identifying eligible care and support needs for adults and carers
  • Develop, implement and review care and support plans that promote independence, well‑being and community inclusion
  • Complete assessments relating to hospital admission, discharge planning, community living, and mental health needs
  • Support individuals to access appropriate services, including domiciliary care, reablement, residential care, equipment, adaptations and direct payments
  • Work with adults experiencing mental health difficulties, contributing to assessment, care planning and recovery‑focused intervention
  • Complete or contribute to Care and Treatment Plans under the Mental Health (Wales) Measure 2010
  • Work in partnership with health professionals within multi‑disciplinary teams
  • Where qualified, undertake duties as an Approved Mental Health Professional (AMHP) and contribute to Mental Health Act assessments

Requirements

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Care Wales.
  • Experience: 2 years post-qualified experience working in a statutory setting. 
  • Legislation Knowledge: Strong understanding of the Mental Health Act 1983, Mental Capacity Act 2005, Social Services and Well‑being (Wales) Act 2014, safeguarding procedures, and the Mental Health (Wales) Measure 2010.
  • Skills: Excellent assessment, communication, and risk‑management skills, with the ability to work effectively in a fast‑paced multidisciplinary environment.
  • Driving Licence: Full UK driving licence and access to a vehicle is often required for community visits.
