Community Support Coach (Lynn, MA)

• →Provides support for individuals in their home and/or within the community.
• →Works on goals and skill acquisition with the individual in areas such as communication, social and recreation, ADL, safety skills while promoting independence, self-determination, and self-advocacy
• →Maintains regular communication with families, Program Director and other professionals involved with the family, when appropriate.
• →Works with families regarding intervention techniques if applicable.
• →Assists individuals and families in accessing resources that might be beneficial.
• →Respects human and legal rights of individuals served.
• →Completes all necessary paperwork such as progress notes, incident reports, time sheets and required DDS progress reports throughout the year.
• →Attends all annual mandatory trainings required by the program as well as Northeast Arc.

• High school diploma or equivalent. Bachelor's degree in human services or another related field preferred.
• Minimum 1 year of experience working with individuals with disabilities.
• Strong interpersonal skills and community skills
• Proficient technology skills for documentation, communicating, and clocking in and out while working.
• Valid driver's license
• Reliable personal vehicle required for client transportation.
• Ability to stand for periods of time.

The Guided Personal Support (GPS) Program is made up of a combination of SSQUAL contracts (Enhanced Family Support for Adults) from the Massachusetts Department of Developmental Services (DDS) that focus on preparing individuals on acquiring skills in different aspects of their lives to be more independent and live their best life. All the referrals in this program come from DDS.
