Direct Support Professional

• Has direct responsibility for the guidance of the adult(s) in Day Support or Community Engagement.
• Is responsible for the physical care of individuals.
• Performs activities and duties to help individuals develop acceptable habits and attitudes.
• Assists with the management of individual behavior, including helping the individuals meet the outcomes of any service or behavioral plan.
• Ensure that the daily needs of the individuals served are met fully.
• Escorts individuals to and from community activities.
• Conduct a head count at the beginning, ending, and periodically throughout the day to verify the presence and well-being of individuals.
• Follow established procedures for reporting behavioral changes or other concerns, cleanliness, physical injuries/conditions, and demeanor.
• Complete data collection in accordance with the person-centered plan,
• Perform required housekeeping duties to assure a clean, safe, and healthful environment.
• Attend staff meetings and in-service training as assigned.
• Plan and implement special activities, and
• Complete special projects and other activities assigned.

Requirements:

• Have a high school diploma, or GED, or 2 years of documented experience working with a family member, a friend, or other persons with intellectual disabilities in a nursing home, facility, group home, or in-home setting.
•  This position requires the successful completion of criminal history and central registry background investigations
• Valid Driver's License
