About the role

We are currently recruiting for a 0.9 FTE (9 days/fortnight) 12-month fixed term Care Partner based in Bacchus Marsh with travel to main office in Ballarat once a month.

As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities. Your days will be filled with meaningful interactions, as you:

• Deliver high-quality, person‑centred care to clients in their homes and communities
• Conduct comprehensive assessments, develop care plans and coordinate services
• Empower clients and carers to make informed choices about their care
• Ensure compliance with aged care quality standards and organisational policies
• Maintain accurate case notes, documentation and client confidentiality
• Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required

About you 

You are compassionate, adaptable, and genuinely committed to making a positive difference in people’s lives. With strong communication skills and a person-centered approach, you thrive in a role where every day brings new opportunities to support, empower, and connect with others.

• Diploma qualification in a relevant field or higher (e.g. Advanced Diploma, Bachelor's degree or above)
• Experience as a Care Partner or Case Manager within Support at Home, Home Care Packages, or the Commonwealth Home Support Programme
• Excellent communication and computer skills
• Understanding of aged care reforms and quality standards
• Ability to work autonomously and manage competing demands
• Commitment to customer service, cultural diversity, and workplace safety
• Willingness to travel as required and a current Victorian driver’s licence
• Satisfactory National Police Check

• Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP)
• Experience working with people from diverse backgrounds
