Responsibilities

- →Supporting people to maintain recovery goals, healthy lifestyles, and positive routines.
- →Signposting and facilitating access to housing, employment, education, volunteering, health services, mutual aid groups, and wider community resources.
- →Identifying and responding to relapse risks, wellbeing concerns, and safeguarding issues.
- →Supporting rapid re-engagement with treatment services when required.
- →Maintaining accurate records and contributing to service quality and continuous improvement.
- →Providing support and guidance to Peer Support Volunteers.
About the Role

You will maintain regular contact with individuals after they leave treatment, providing a mixture of telephone, online, face-to-face, and group-based support. You will help people identify and achieve their recovery goals, connect with community resources, and develop strong support networks that promote independence and wellbeing.
We are looking for someone who is passionate about supporting people to achieve lasting change and who can build trusting, professional relationships with service users and partner organisations.
You will have:
- Experience supporting individuals with substance misuse, mental health, wellbeing, or recovery-related needs.
- Knowledge of recovery principles, relapse prevention, safeguarding, and person-centred approaches.
- Experience delivering support in a variety of settings, including face-to-face, telephone, online, and group environments.
- Strong organisational skills and the ability to manage caseloads and maintain accurate records.
- Excellent communication and relationship-building skills.
